Most Australians know about the health checks that arrive automatically: the ones that show up in the letterbox or get raised at a routine appointment once you hit a certain age. Fewer know there’s a free, Medicare-funded health check specifically for people aged 45 to 49: not because it’s hidden, but because nobody sends you a reminder for it. You have to know it exists to ask for it.

If you’re in this age bracket, here’s what it actually involves and whether it’s worth booking.

What is the 45–49 health check?

It’s a Medicare-funded health assessment specifically for people aged 45 to 49 who are considered at risk of developing a chronic disease. Unlike some health checks that apply to everyone in an age group automatically, this one is based on your GP’s clinical judgement about whether you fit that ‘at risk’ description: which, in practice, covers a fairly wide range of people, since risk factors are common and often overlap.

Am I at risk? What does that actually mean?

Being eligible doesn’t mean something is already wrong. Your GP is looking at a combination of lifestyle and biomedical risk factors, including things like:

  • Smoking
  • Limited physical activity
  • Diet and nutrition
  • Alcohol use
  • High cholesterol
  • High blood pressure
  • Blood sugar levels outside the typical range
  • Weight

What happens during the assessment?

The structure varies a little by practice, but it typically involves a conversation about your health history, lifestyle, and family history of chronic conditions, along with some straightforward checks: blood pressure, weight, and a general review of relevant risk factors. Part of this may be conducted with a practice nurse before you see your GP, with the GP then reviewing the results and talking through anything relevant with you directly.

It’s a genuinely practical conversation, not a lecture. The goal is identifying what’s manageable now, while it’s easiest to actually change something.

Is it really free?

Yes. As a Medicare-funded health assessment, this check is bulk-billable, meaning it shouldn’t cost you anything out of pocket where your GP bulk bills the service.

How often can I have one?

This assessment is available once only under the relevant Medicare items. It’s a one-off opportunity in this specific age window, which is part of why it’s worth actually booking rather than assuming it’ll come around again.

What happens after the assessment?

If any risk factors come up, your GP will usually talk through practical next steps: this might mean lifestyle changes, further monitoring, a referral, or simply a plan to revisit something at your next regular appointment. Nothing about this assessment is designed to alarm you; it’s designed to give you and your GP an accurate, current picture to work from.

Should I book one?

If you’re between 45 and 49 and haven’t had a conversation with your GP specifically framed around chronic disease risk, it’s worth asking about at your next appointment or booking one specifically for this purpose. Given it’s a one-off assessment during a fairly narrow window, there’s little downside to raising it, even if it turns out your GP feels a full assessment isn’t clinically necessary for you right now.
