Mental Health & Chronic Care

Mental Health & Chronic Care
in Cannonvale

Health rarely fits neatly into one box.

A long-term condition can affect sleep, mood and energy. Stress can make it harder to keep up with medication, appointments or the day-to-day work of managing your health. Sometimes the reason you book is emotional. Sometimes it is physical. Quite often, the two overlap.

You do not need the perfect label.

How We Support You

What GP-led ongoing care can look like.

  1. 1

    Talk about what has changed

Start with what you have noticed, what is difficult and what you would like help with. You do not need a prepared diagnosis.

  2. 2

    Look at the wider context

Your GP may consider symptoms, health history, medications, physical health, mental wellbeing, previous results and current circumstances.

  3. 3

    Agree on the next priority

Sometimes the next step is simple. Other times it may involve further assessment, a structured plan, referral or ongoing GP review.

  4. 4

Bring in other care when it is useful

Where clinically appropriate, your GP may coordinate with other healthcare professionals rather than leaving you to connect every part of care yourself.

  5. 5

    Review and adjust over time

Mental health and chronic conditions can change. Follow-up allows the plan to change with you rather than staying fixed.

Start With What Is Affecting You

The useful starting point is not choosing the perfect label. It is giving your GP enough information to understand what has been happening and what is getting in the way of everyday life.

  • Something has changed in your mood, sleep, concentration or ability to cope.
  • You want to talk about mental health but are not sure what kind of support you need.
  • Someone has suggested a Mental Health Care Plan or Mental Health Treatment Plan.
  • You have concerns about attention, impulsivity or concentration and want to discuss ADHD assessment.
  • You are managing a chronic disease or chronic illness and would like more structured ongoing care.
  • Your diabetes or blood pressure needs review or follow-up.
  • You want to discuss weight in the context of your wider health rather than follow a quick-fix program.

Mental Health Support: Somewhere to Start

Some mental health appointments begin with a clear diagnosis. Many do not. You may simply know that you are worrying more, sleeping badly, losing motivation, feeling overwhelmed or finding it harder to concentrate. A GP appointment gives you a place to talk through those changes without needing to diagnose yourself first.

A mental health assessment may look at what you are experiencing, how long it has been happening, how it affects daily life, relevant physical health, medications, previous care and your current circumstances.

Chronic Care: Looking Beyond the Next Appointment

Chronic does not necessarily mean that you feel unwell every day. It means your health needs an ongoing view. A chronic condition can involve regular monitoring, medication review, test results, lifestyle changes and care from more than one health professional. Good chronic care helps connect those pieces so each appointment does not feel like starting again from the beginning.

  • Regular monitoring
  • Medication review
  • Test results

Connected ongoing care

  • Lifestyle changes
  • GP review
  • Other health professionals

For eligible patients, structured care may include a GP Chronic Condition Management Plan, the current Medicare pathway that replaced the older GP Management Plan and Team Care Arrangements framework in 2025. Eligibility and the right care pathway depend on the individual patient, so the dedicated Chronic Disease Management page explains that process in more detail.

Mental and Physical Health Often Meet in the Middle

A chronic illness can be tiring. It can change routines, create uncertainty and add another layer of appointments or decisions to everyday life. At the same time, stress, low mood, anxiety or poor sleep can make a physical condition feel harder to manage. A GP may need to consider several things together:

Sleep and energy

Stress or treatment fatigue

Changes in mood or motivation

Medication questions or side effects

Work, family or caring responsibilities

Changes in appetite, movement or daily routine

Worry about a long-term health condition

The point is not to turn every health concern into a mental health problem, or every emotional concern into a physical one. It is to make sure your GP has room to consider the connections when they are relevant.

Fees, Medicare and Referrals

Coral Coast Doctors bulk bills eligible Medicare consultations where the service is eligible under Medicare. Some services may not be covered by Medicare or may involve an out-of-pocket fee.

Mental Health Treatment Plans and GP Chronic Condition Management Plans have their own Medicare requirements. If another health professional becomes involved in your care, that provider sets their own fees and may charge a gap even when a Medicare benefit is available.

If you are unsure what may apply to your appointment, ask reception before attending.

Mental Health & Chronic Care FAQs

What mental health services are available through Coral Coast Doctors?

Coral Coast Doctors provides GP mental health assessment and ongoing support, with dedicated pages explaining general mental health care, Mental Health Care Plans / Mental Health Treatment Plans and ADHD Assessment & Management. The most appropriate pathway depends on your individual situation.

Can I talk to a GP about mental health without having a diagnosis?

Yes. You can book because something has changed, you are finding things difficult or you want to understand what may be going on. A GP can assess the wider picture and discuss appropriate next steps.

What is the difference between this page and the Mental Health page?

This page is the main hub for both mental health and chronic care. The dedicated Mental Health page goes into more detail about mental-health concerns, GP assessment and ongoing mental-health support.

What is a Mental Health Care Plan?

Mental Health Care Plan is the term many patients search for. The current Medicare terminology is Mental Health Treatment Plan. A GP needs to assess whether that structured pathway is clinically appropriate and whether eligibility requirements are met.

What is chronic disease management?

Chronic disease management is ongoing care for long-term health conditions. For eligible patients, it may include a GP Chronic Condition Management Plan, which can help structure goals, reviews and coordination with other care where appropriate.

Can Coral Coast Doctors help with diabetes and high blood pressure?

The website includes dedicated Diabetes Management and Hypertension Management service pages, and supplied doctor profiles document relevant chronic-disease experience across the team.

Is medical weight management the same as a diet program?

No. The dedicated Medical Weight Management page is framed as GP-led healthcare. It considers weight in the context of overall health, including medical history, medications, sleep, nutrition, physical activity, mental wellbeing and other health conditions.

Are mental health and chronic care appointments bulk billed?

Eligible Medicare consultations at Coral Coast Doctors are bulk billed where the service meets Medicare requirements. Specific plans, referrals and external providers can have separate eligibility or fee rules, so ask reception if you are unsure.

You Do Not Need the Perfect Label Before You Book

You might be dealing with a mental health concern, an ongoing physical condition, or a mix of both. You might need a structured plan. You might simply need a GP to help you make sense of what has changed. That is enough to start.
